Entry form Downloads

Below are 2 downloadable versions of the Entry form, the Full version and the Mail In section only.

Full Version: This file has been removed from the site due to its size (1.3MB). It is available by special request.
Mail In Only Version: Contains just the section of the entry form that you fill out and mail in. Download this if you need to send they entry form in with a cheque or parents signature.
It is in PDF format, use Adobe Acrobat to open this file.

NOTE: If you have problems with images displaying briefly and then disappearing turn off Smooth Text and Monocrome Images. From the File menu select Preferences -> General... And untick Smooth Text and Monocrome Images.
